HELP TO BUILD BETTER COMMUNITIES FOR SENIORS Assist private-pay nursing facility residents relocating from a nursing facility back to a community setting. In addition, also assist private pay consumers residing in community settings with developing a support plan to remain in their homes A minimum of a bachelor's degree or a master's degree from an accredited college or university in registered nursing, social work, gerontology or related human services field is required. Experience in care coordination and case management is essential AND previous relevant employment experience at a long-term care facility, assisted living, hospital, home care or lead agency is required. Salary range $21 $24 per hour Must have experience with activities such as discharge planning, home and community-based service coordination or person-centered support planning Valid MN driver's license, proof of insurance and reliable car required. A degree in social work requires licensure or demonstrated progress/intent on securing licensure. A degree in nursing requires a current MN RN License.
